ENROLLMENT
HOW TO ENROLL YOUR CHILD at SUMCNS:
1. Call us at 513 563 8278 to obtain current information on openings.
2. Complete and mail to us the appropriate application form:
Yellow heading is for NEW families.
White heading is for current families who are re enrolling
Blue heading is for former/ church/ and waiting list families
3. Mail these forms along with your check for $60 registration fee per child to:
SUMC Nursery School and Kindergarten

4. You will receive an acknowledgement note containing your child's class
placement.
5. Before your child's first day of attendance, please print out:
Child's Medical Statement and have your doctor's office complete that. Medical forms
must be less than 12 months old on the first day of attendance.
Ohio Enrollment Form: This must be complete and in our hands for your child to
attend the first session.
